Subscription Cancellation and Recurring-Charge Dispute Log Template

Canceling a service, revoking an automatic payment, and disputing a charge are separate actions. A complete log preserves evidence for each one.

What to know

  • Canceling a service, revoking an automatic payment, and disputing a charge are separate actions. A complete log preserves evidence for each one.
  • Preserve the original terms, cancel through the correct billing channel, revoke automatic payments separately when necessary, and monitor the account afterward.

Create a record as soon as the subscription begins.

Track:

  • Merchant
  • Product
  • Account email
  • Trial period
  • Renewal date
  • Price
  • Billing frequency
  • Cancellation method
  • Cancellation request
  • Confirmation
  • Final charge
  • Refund
  • Bank or card dispute

FTC recommends recording trial terms, marking the cancellation deadline, and monitoring statements for charges that continue after cancellation.

Preserve the original offer

Save:

  • Checkout page
  • Trial terms
  • Renewal terms
  • Cancellation policy
  • Refund policy
  • Confirmation email
  • Price and frequency
  • Minimum commitment
  • Included products

Take screenshots showing:

  • Date
  • Domain
  • Price
  • Renewal language
  • Required notice

A checkout page can change later.

The merchant’s current cancellation terms may not prove what the user agreed to months earlier.

Calendar the deadlines

Record reminders:

  • Seven days before trial ends
  • Three days before renewal
  • Renewal date
  • Contract notice deadline
  • Expected final bill
  • Refund deadline

FTC advises marking trial expiration dates and reviewing statements so unwanted charges are discovered promptly.

Do not depend only on the merchant’s promised reminder.

Cancellation log

| Date and time | Channel | Action | Representative | Confirmation | Effective date | |---|---|---|---|---|---| | | | | | | |

Possible channels include:

  • Account dashboard
  • Application store
  • Email
  • Telephone
  • Chat
  • Postal mail
  • Retail location

Cancel through the party actually billing the subscription.

A subscription purchased through Apple, Google, a cable company, or another marketplace may not be controlled from the service’s own website.

Save proof of cancellation

Preserve:

  • Confirmation page
  • Confirmation number
  • Email
  • Chat transcript
  • Call recording where lawful
  • Postal tracking
  • Screenshot showing canceled status
  • Effective date
  • Access end date

After a telephone call, send a written message:

This confirms my request today to cancel the subscription effective immediately and prevent further renewal charges.

Ask the merchant to correct any misunderstanding in writing.

Cancellation and account deletion are different

Canceling billing may not:

  • Delete the account
  • Remove saved data
  • End access immediately
  • Cancel a financing agreement
  • Return rented equipment
  • Cancel another bundled service

Account deletion may not stop billing when the subscription is managed through another platform.

Verify each action separately.

Revoking automatic bank payments

When recurring debits use a bank account, tell both:

  1. The merchant
  2. The bank or credit union

CFPB advises revoking authorization with the company and notifying the financial institution, preferably with written follow-up.

Record:

Authorization revoked:
Merchant notified:
Bank notified:
Written notice sent:
Stop-payment fee:
Stop-payment expiration:
Future debit monitored:

Stopping a payment does not automatically eliminate a legitimate underlying contractual debt.

Credit-card dispute log

| Charge date | Statement date | Amount | Merchant contacted | Written dispute | Result | |---|---|---:|---|---|---| | | | | | | |

For federal credit-card billing-error protection, CFPB advises sending the written dispute to the billing-inquiries address within 60 calendar days after the statement containing the error was sent.

The issuer generally must acknowledge the dispute within 30 days unless it resolves it sooner.

Keep:

  • Copy of letter
  • Statement
  • Cancellation evidence
  • Delivery tracking
  • Issuer response

Calling first can help, but a telephone call may not preserve every written-dispute right.

Debit-card and bank-transfer disputes

Debit-card, ACH, and electronic-fund-transfer rules differ from credit-card rules.

Notify the financial institution promptly.

Record:

  • Date first reported
  • Whether authorization had been revoked
  • Provisional credit
  • Investigation deadline
  • Written confirmation
  • Final determination

CFPB says a financial institution must begin investigating qualifying electronic-transfer errors after receiving timely notice and may require written confirmation in some circumstances.

Merchant contact log

| Date | Person | Channel | Case number | Statement | Promise | Deadline | |---|---|---|---|---|---|---| | | | | | | | |

Record exact statements.

Avoid summaries such as:

Support said it should be fine.

A useful record is:

Agent 1847 said the charge of twenty-nine dollars and ninety-nine cents would be refunded within five to seven business days under case 88241.

Monitor after cancellation

Check:

  • Merchant account
  • Card statement
  • Bank account
  • Application-store subscriptions
  • Email
  • Credit report where financing was involved

Continue monitoring for at least several billing cycles when the merchant previously charged incorrectly.

Final resolution

Obtain written confirmation that:

  • Subscription is canceled
  • Automatic renewal is disabled
  • Final charge is correct
  • Refund was issued
  • Bank dispute is closed
  • Equipment return is credited
  • No balance remains

Record the date the refund actually posts.

A promised refund and a posted refund are two different financial species.

Minimal spreadsheet header

Merchant | Service | Signup | Trial End | Renewal | Price | Payment Method | Cancellation Date | Confirmation | Final Charge | Refund | Dispute Deadline | Resolution

Bottom line

Preserve the original terms, cancel through the correct billing channel, revoke automatic payments separately when necessary, and monitor the account afterward.

The log exists because “cancel anytime” frequently means “cancel through a hidden menu, then document the conversation for the next three billing cycles.”
