Vehicle Accident Evidence, Insurance, and Repair Log Template

A collision can produce separate police, insurance, medical, repair, rental, lender, and state-reporting records. One chronological log keeps facts, deadlines, and promises together.

What to know

  • A collision can produce separate police, insurance, medical, repair, rental, lender, and state-reporting records. One chronological log keeps facts, deadlines, and promises together.
  • Record the scene, people, vehicles, insurance, police, injuries, estimates, repairs, expenses, and every promise.

Create the record as soon as it is safe.

First:

  1. Move away from active traffic when possible.
  2. Call emergency services for injuries, danger, fire, blocked roads, or legal reporting requirements.
  3. Do not move an injured person unless immediate danger requires it.
  4. Exchange information.
  5. Document the scene without obstructing responders.

NAIC recommends collecting driver, vehicle, insurance, witness, officer, location, weather, road, photograph, and diagram information after a crash.

Incident identity

Incident ID:
Date:
Time and time zone:
Exact location:
Police agency:
Report number:
Tow company:
Vehicle location:
Insurer:
Claim number:
Attorney or adviser:
Log last updated:

Write facts rather than conclusions about fault.

Example:

Vehicle B entered the intersection from the west while my signal displayed green.

Avoid:

The reckless idiot deliberately hit me.

One version is evidence. The other is an understandable but less useful review.

Driver and vehicle exchange

| Field | Your vehicle | Other vehicle | |---|---|---| | Driver name | | | | Address | | | | Telephone | | | | License number | | | | Insurer | | | | Policy number | | | | Plate | | | | VIN | | | | Make and model | | | | Registered owner | | |

NAIC advises obtaining the other driver’s name, address, insurer, policy details, vehicle description, plate, and license information.

Photograph documents only with permission and where lawful.

Do not post them publicly.

Witness record

| Name | Telephone | Email | Location | What observed | Statement saved | |---|---|---|---|---|---| | | | | | | |

Ask witnesses for contact information before they leave.

Do not coach them.

Record their own words and where they were positioned.

Scene photographs

Capture safely:

  • All vehicles from four corners
  • License plates
  • Damage close-ups
  • Vehicle positions
  • Traffic signals and signs
  • Lane markings
  • Skid marks
  • Debris
  • Weather
  • Road surface
  • Nearby businesses or cameras
  • Property damage
  • Visible injuries where appropriate
  • Airbag deployment
  • Child restraints

NAIC recommends photographs or a scene sketch.

Do not stand in traffic to obtain the perfect angle.

Scene diagram

Record:

  • Direction of travel
  • Lane
  • Intersection
  • Traffic control
  • Initial impact
  • Final positions
  • Pedestrians
  • Bicycles
  • Obstructions

Use north or another fixed orientation.

Mark the diagram as your recollection unless it is taken from an official report.

Police record

Officer:
Badge:
Agency:
Report number:
Citation issued:
Report request date:
Report received:
Correction requested:

Ask when and where the report will be available.

A police report may contain errors or preliminary information.

Preserve the original before requesting a correction.

State reporting

State motor-vehicle reporting rules differ.

For example, California currently requires an SR-1 within 10 days when anyone is injured or killed or property damage exceeds one thousand dollars.

Record:

Jurisdiction:
Reporting threshold:
Deadline:
Form:
Submitted:
Confirmation:

Do not borrow another state’s threshold from a search result because both jurisdictions use roads.

Medical record

| Date | Provider | Symptom | Diagnosis | Treatment | Work restriction | Bill | |---|---|---|---|---|---|---| | | | | | | | |

Seek emergency help for serious symptoms.

Some injury symptoms appear or become clearer after the immediate crash.

Record:

  • Headache
  • Neck or back pain
  • Dizziness
  • Numbness
  • Bruising
  • Sleep changes
  • Reduced movement

Do not exaggerate or minimize.

Use the clinician’s exact diagnosis rather than inventing one for the spreadsheet.

Child restraints

Record:

  • Seat manufacturer
  • Model
  • Serial number
  • Manufacture date
  • Position
  • Occupied or unoccupied
  • Crash severity
  • Insurer decision
  • Replacement

NHTSA recommends replacing child restraints after moderate or severe crashes. A restraint does not automatically require replacement after a crash meeting all of NHTSA’s minor-crash criteria.

Follow the child-seat manufacturer’s instructions, which may be stricter.

Insurance contact log

| Date and time | Insurer | Representative | Channel | Case number | Promise | Deadline | |---|---|---|---|---|---|---| | | | | | | | |

Record:

  • Liability claim
  • Collision claim
  • Medical payments
  • Uninsured motorist
  • Rental
  • Towing
  • Storage
  • Deductible
  • Total-loss review

NAIC recommends preserving receipts, conversations, and submitted claims.

Damage and estimate record

| Area | Visible damage | Photo | Shop estimate | Insurer estimate | Supplement | |---|---|---|---:|---:|---:| | | | | | | |

Ask the repair shop to distinguish:

  • Visible damage
  • Hidden damage found during disassembly
  • Safety-system calibration
  • Structural measurement
  • OEM versus alternative parts
  • Paint blending
  • Alignment
  • Sensor calibration

Do not compare estimates only by final total.

Repair record

Shop:
Drop-off:
Repair authorization:
Estimate:
Supplement:
Parts:
Calibration:
Completion:
Warranty:
Final invoice:
Pre-delivery inspection:

Photograph the repaired vehicle.

Test:

  • Lights
  • Warning indicators
  • Steering
  • Brakes
  • Driver-assistance functions
  • Windows
  • Locks
  • Air conditioning
  • Alignment

Report unresolved safety symptoms immediately.

Rental and transportation costs

Track:

  • Rental
  • Rideshare
  • Public transit
  • Towing
  • Storage
  • Mileage
  • Parking
  • Child-seat replacement
  • Missed work

Preserve receipts.

Coverage depends on the policy, fault rules, limits, and claim status.

Insurance-report history

Auto claims can appear in specialty consumer reports used by insurers.

CFPB says the C.L.U.E. system can report up to seven years of auto claims. Consumers can obtain and dispute inaccurate specialty reports.

After resolution, review the record when inaccurate claim information could affect future insurance.

Minimal spreadsheet header

Incident | Date | Driver | Vehicle | Insurer | Police Report | Witness | Injury | Tow | Estimate | Repair | Rental | Claim Status | Final Resolution

Bottom line

Record the scene, people, vehicles, insurance, police, injuries, estimates, repairs, expenses, and every promise.

A collision lasts seconds. The administrative afterlife can last months, because civilization has apparently determined that damaged metal requires seven independent institutions and 46 document versions.
